SUMMER DANCE CAMP REGISTRATION

Please use the form below to register your student for summer dance camp. You will have the ability to register one student at a time. Once the form is completed, you will be allowed to proceed to the next step for payment of the corresponding summer camp fee. Payment is required upon registration to hold your spot in the camp. Please have debit or credit card information ready at time of submitting your registration form. All payments are nonrefundable. Please repeat these same steps as necessary to register additional students. Registration is based on a first come, first served basis, and camps will close once capacity is reached. Waiver of Liability

As legal parent or guardian, I release and hold harmless Walker School of Ballet, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Walker School of Ballet, its owners and operators or in route to or from any said premises. Any person entering the premises waives all civil liability against this premises owner and operator for any injuries caused by the inherent risk associated with contracting COVID-19 at public gatherings, except for gross negligence, willful and wanton misconduct, reckless infliction of harm, or intentional infliction of harm, by the individual or entity of the premises.

I hereby declare any Physical/mental conditions or restrictions, and/or declare the participant to be in good physical and mental health.

Social Media - Picture Permission *

I give Walker School of Ballet permission to use my child(s) picture for the purpose of advertising on the studio website, social media and/or informational brochures. If for any reason I do not feel comfortable with this, written notice below to the school will ensure my child's picture will not be used for specified reasons.
